1. What is America Living Will Registry? 
   America Living Will Registry provides anytime, anywhere access to your medical advance directives. Registrants carry an ID card with a toll free emergency number and web address. Emergency medical personnel may obtain copies of your directives either on-line or via fax.
   
2. Why it is so important to store these documents in ALWR's database?
   You will have peace of mind knowing that in the event you are unable to speak for yourself or make your own medical decisions, ALWR is available to send your documents to your health care provider so your wishes are honored.  You are also relieving your family of the stress of locating your documents to be sure your wishes are fulfilled.

4. How do I store my advance directive with ALWR?
   Once you have registered, there are two options:
   1) Fax the documents to 1-813-354-3627
   2) Mail the documents to:

5. Once my documents are on the site, am I able to view them?
   Yes, upon membership, you will be issued an ID card and PIN number.  At this time, you will be able to view your documents.
   
6. How will my Health Care Provider be able to access my documents?
   The Health Care Provider will need the information on your ID card to access your files and will simply follow the directions on the card.
   
7. What if I want to change my advance directive?
   Just send ALWR your new advance directive to be stored in our database.  Please note that your previous documents will be deleted.  ALWR will only keep your latest directive on file.  There is no additional charge for this service and you may update your documents as often as you wish.
   
8. What if I lose my ID card?
   You may request a replacement card by visiting our website and completing the Duplicate ID card information.  Your card will be mailed to you within 24 hours.
